Tabling Request Form
Thank you for requesting Center for Campus Wellness presence at your tabling event. Please note that we need 3 weeks lead time to guarantee we can fulfill your request.
CCW is available to table at resource fairs, events, and orientations. We staff our tables with CCW professional and student staff and provide information on our programs and services, as well as swag.
If you would like to request CCW for a tabling event, please fill out the form below.
Please note: we need at least 3 weeks to process workshop requests. Priority is given to University of Utah student audiences
(e.g., classroom requests, student groups). We may be able to accommodate other requests
on a case-by-case basis. Please email wellness@sa.utah.edu with any questions!
